Immunization Clinic at Holy Trinity

Ottawa Public Health will be holding an Immunization Clinic at Holy Trinity on Monday, October 23, Tuesday, October 24, and Wednesday, October 25, 2107.  These vaccines are publicly-funded, and available at no cost to you.

On Monday, October 16, your child received the vaccine consent form(s) from Ottawa Public Health.  Your child needs to return the signed consent form(s) to the school immunization clinic in order to receive the vaccines.  

All students in grade 7 will receive consent forms for the following:

  1. Hepatitis B

  2. Meningococcal Disease Vaccines (Menactra)

  3. Human Papillomavirus (HPV) vaccine

* Please note that students in grade 8 and up who need to receive the grade 7 vaccines (to complete series etc) should present themselves the morning of the clinic, before homeroom, sign their name and room number, then immediately return to class.  These students (if consent is on file) will be called to the clinic throughout the day.

Students need a signed consent to get this vaccine. You are encouraged to talk with your child about the vaccines; and ideally both you and your child should sign the consent form.

On the day of the school clinic, remind your child to:

  • Eat a good breakfast.

  • Bring an extra juice or a snack to school.

  • Wear short sleeves or a loose shirt so that his or her upper arm is exposed easily, without undressing.

  • Bring the signed consent forms with them to the school clinic

Students who miss a clinic will be able to receive the vaccine at the next school clinic.
